Registration for high school students is a two-step process

STEP ONE:

  1. Click on one of the registration links above, create a new account or sign in to your enrollment account if you have one, and begin completing the registration form
  2. The following documents will need to be uploaded on your device (via scan, dropbox, or photo library)
    1. For proof of your child's identity, please upload one (1) of the following:
      1.  birth certificate
      2.  current passport
      3.  current Permanent Resident Alien Card
    2. For proof of Parent's identity, please upload one (1) of the following:
      1. current driver's license
      2. current State of CT issued ID
      3. current passport
      4. current Permanent Resident Alien Card
    3.  For primary proof of residency, please upload (1) of the following:
      1.  current lease - signed and dated by both tenant and landlord
      2.  current mortgage statement
      3.  current house property tax bill
    4. For secondary proof of residency, please upload (1) of the following:
      1.   pages 1 and 2 of a current electric or gas utility bill showing "service address"
      2.  current auto insurance showing name and current address
      3. current pay stub showing name and current address
      4. current determination letter showing name and current address from BHA, DSS, SS, SSDI, or Husky 
    5. Current Physical
    6. Current Immunization Record
    7. Dental Form
  3. Complete online form

STEP TWO:
Once you have completed the registration form and uploaded the required documents, your child's school will contact you to select classes.


If you have questions, concerns, or do not have the documentation listed above, please contact the Central Registration Support Line Monday - Friday from 8:00 am to 4:00 pm at  860-584 7033.

Medical clearance must be obtained prior to the first day of in-person school in order to attend.  Please upload your child's medical information to the online registration form or submit to the Bristol-Burlington Health District via email or fax to the School Health Services Coordinator.  The email address is: [email protected].  The fax number is 860-584-3814.  Parents can also have their child's healthcare provider send the medical information via fax to the attention of the School Health Coordinator at the Bristol-Burlington Health District. Parents will be contacted directly if there are any questions or concerns about the medical information submitted.

The following documents are required to obtain medical clearance:

  1.  Current Physical (download #1 below)
  2.  Immunization Record (download #1 below)
  3.  Dental Form (download #2 below)
