State Mandated Health Requirements
Sixth Grade Students
The following State mandated health requirements must be on file in the Health Room before entrance into seventh grade:
- Health Assessment/Physical dated after completion of the fifth grade.
- MMR booster.
- Hepatitis B vaccine, at least one dose.
- Varivax (chickenpox) vaccine or statement from a physician confirming the child already had chickenpox.
Please try to have this information on file in the Health Room before the last day of school.
Seventh Grade Students
The Hepatitis B vaccine series (3 doses) must be complete and on file in the Health Room before entrance into the eighth grade.
