CONCUSSION EDUCATION PLAN FOR PARENTS AND STUDENT-ATHLETES

Concussions are on the forefront of the news these days. The CIAC is mandating education for parents of high schools and middle school students who want to participate interscholastic and intramural sports. Guardians and students must sign a Student & Parent - Concussion Education Plan & Consent Form.

The education can be done through a video or handout. The handout, a video link, and the consent form (English and Spanish) are linked below. If a student is interested in participating, they can view the video and access the literature online. If a parent does not have computer access, we can make the handout available in hard copy.
